NHS Referrals
Here at the forum we can accept NHS patients that have been referred to us by their dentist.
If you have a child under 18 they may be entitled to NHS funded orthodontic treatment. If you would like someone under 18 to be seen by us on the NHS then we would require a referral from their regular dentist requesting an assessment. Please be aware though that waiting times for NHS orthodontic treatment can be quite long due to the demand for the service.
This is something you can discuss with your regular practice at you or your child’s next visit with them. Please note we cannot see children for an NHS assessment without a referral from their dentist.
If your child is on our waiting list for NHS treatment but you are interested in getting them into treatment quicker privately, then please contact the Practice Manager Oliver by phone on 01427 615117 or via email at ojackson@theforumpractice.com. We do offer private treatment at a specially discounted rate just for children who are either on the NHS waiting list or who have failed to meet the NHS criteria for treatment.

Patients are given the choice to be seen at one of several locations, depending on which is most convenient for them.
Please note that all referrals must be sent with clear instruction for treatment, an up to date medical history, and an appropriate x-ray.
